qFibrosis® Revealing New Insights into Treatment Response with Resmetirom in the MAESTRO-NASH Phase 3 Study
qFibrosis® once again takes center stage in advancing our understanding of fibrosis in MASH.
A recent Journal of Hepatology publication, “Quantitative regression of qFibrosis with resmetirom: Exploratory histologic endpoints from the MAESTRO-NASH Phase III clinical trial,” highlights how AI-driven digital pathology is reshaping fibrosis evaluation.
In the MAESTRO-NASH Phase 3 trial, qFibrosis® was used to assess fibrosis changes in patients treated with resmetirom – the first approved therapy for MASH. The study shows that qFibrosis® detects treatment-related changes with greater sensitivity than conventional histology alone.
By moving beyond ordinal staging, qFibrosis® enables a continuous and more detailed assessment of collagen remodeling. Notably, it identified fibrosis improvements even in patients classified as non-responders, pointing to early or subtle treatment effects that traditional methods may overlook.
The findings also emphasize the value of spatial analysis, with region-specific collagen changes providing additional biological insight and showing alignment with non-invasive modalities such as MRE and VCTE.

Together, these results support a shift toward more sensitive, quantitative, and biology-driven approaches to assessing fibrosis response.
